Ambulatory Care Center Achieves Gold Certification

Doha: The Ambulatory Care Center (ACC) has become the second Hamad Medical Corporation (HMC) facility to achieve Certification for Excellence in Person-Centered Care by Planetree International, and the first hospital in Qatar to be awarded the gold Certification after a rigorous evaluation process.

Last year, Enaya Specialized Care Center became the first facility in Qatar to achieved Person-Centered Care Certification from Planetree International. The Communicable Disease Center, Home Health Care Services and the Private Nursing Services are currently undergoing the Certification process.

Opened in 2017, the ACC is an innovative facility that offers a new approach to surgical care in Qatar using advanced clinical and surgical practices offered within the same day and in a single dedicated location. The team underwent a rigorous evaluation by the Planetree team in 2020 to achieve the Certification. Planetree International is a not-for-profit organization that partners with healthcare organizations around the world to transform how patient care is delivered.

Nasser Al Naimi, who is the Deputy Chief of Quality at the Center for Patient Experience and Staff Engagement and Director for the Hamad Healthcare Quality Institute said the Planetree Certification represents the highest level of achievement in person-centered care based on evidence and standards and it is a great achievement by the leadership and team at the ACC to reach this level.

“At the very heart of person-centered care is the concept that the patient, their family members and the care provider are partners in their healthcare process and the ACC has demonstrated their commitment to involving patients, family members and staff at every step of the way,” Al Naimi said.

“Planetrees independent certification committee evaluated the Ambulatory Care Center against 26 specific and difficult criteria and determined it has met the standards to be deemed Gold Certified for Excellence in Person-Centered Care,” he added.

He also said that the person-centered care program drives at HMC also helps in identifying areas of improvement, in addition to helping find innovative ways to further enhance the patient experience and engage with patients and the community to ensure they receive the best care possible.

“We look forward to all of our facilities achieving this important milestone and following the lead set by the ACC and also Enaya,” Al Naimi said.

Medical Director of the ACC Dr. Khalid Al Jalham said the criteria used by Planetree to assess the ACC, addressed components of a person-centered healthcare experience, including the quality of patient interactions, access to information, family involvement and the physical environment of care.

“The care we give at the ACC is unique – providing day care surgery, some inpatient surgical care and clinical care in a single dedicated location,” Dr. Al Jalham said.

“Many of our patients are with us for their procedure and are able to recover in the comfort of their own homes. But we are dedicated to ensuring they receive not only the best of care when they are with us, but also in their follow-up care. We worked hard to implement new processes to make sure our patients are supported on their journey,” he added.

He noted that the certification was the only program of its kind that globally and formally recognizes the ability of the team at the ACC to deliver excellence in person-centered care. We strive every day to raise the bar in the care and support provided to patients, the people of Qatar and the staff.

For her part, President of Planetree International Susan Frampton said that the certification was the only award that recognizes excellence in person-centeredness across the continuum of care.

An integral part of the move towards person-centered care and the certification process was the establishment of a Patient and Family Advisory Council (PFAC) at the ACC. The council meets regularly and advises on a number of key issues and improvements at the facility. Member of the ACCs Patient and Family Advisory Council Maryam Al Emadi said it was a great experience to be involved in this important journey for the facility.

الدوحة: حصل مركز الرعاية الطبية اليومية التابع لمؤسسة حمد الطبية على اعتماد التميز في الرعاية المتمحورة حول المريض من منظمة “بلينتري” الدولية ليصبح بذلك ثاني مرافق المؤسسة حصولا على هذا الاعتماد، وأول مستشفى في دولة قطر يحصل على هذا الاعتماد وذلك بعد خضوعه لعملية تقييم وفق معايير ملزمة.

وكان مركز (عناية) للرعاية التخصصية أول مرفق في دولة قطر يحصل على هذا الاعتماد في الرعاية المتمحورة حول المريض من منظمة بلينتري الدولية العام الماضي، ويجري العمل حاليا على حصول مرافق وخدمات أخرى بمؤسسة حمد الطبية على هذا الاعتماد وهي مركز الأمراض الانتقالية، وخدمات الرعاية الصحية المنزلية، وخدمات التمريض الخاص.

ويعد مركز الرعاية الطبية اليومية الذي تم افتتاحه في عام 2017 أحد المراكز الطبية المتقدمة التي تقدم منهجية جديدة للرعاية وخدمات الجراحة بدولة قطر، حيث تعتمد على استخدام أحدث الممارسات الإكلينيكية والجراحية لإجراء عمليات اليوم الواحد الجراحية وتلبية كافة احتياجات المرضى في موقع واحد.

وقد خضع المرفق لعملية تقييم وفق معايير ملزمة من قبل فريق منظمة بلينتري الدولية خلال عام 2020 للحصول على هذا الاعتماد.

تجدر الإشارة إلى أن منظمة بلينتري الدولية، هي منظمة غير ربحية تقيم شراكات مع منظمات الرعاية الصحية في شتى أنحاء العالم بهدف إحداث تغييرات للارتقاء بالرعاية المقدمة إلى المريض.

من جانبه قال السيد ناصر النعيمي نائب الرئيس لقطاع الجودة بمؤسسة حمد الطبية ومدير معهد حمد لجودة الرعاية الصحية إن اعتماد منظمة بلينتري الدولية يمثل أعلى مستوى من الإنجاز في الرعاية المتمحورة حول المريض بناء على الأدلة والمعايير، ويعد الحصول على اعتماد دولي بهذا المستوى بمثابة إنجاز كبير لقادة وفريق العمل بمركز الرعاية الطبية اليومية.

وأضاف ان الرعاية المتمحورة ترتكز حول المريض على مفهوم أن المريض وأفراد أسرته ومقدم الرعاية الصحية هم شركاء في عملية حصول المريض على الرعاية الصحية، وقد أظهر مركز الرعاية الطبية اليومية التزامه بإشراك المرضى وأفراد أسرهم وفريق الرعاية الصحية في جميع مراحل تقديم الرعاية الصحية للمريض.

وقامت لجنة الاعتماد المستقلة في منظمة بلينتري بتقييم مركز الرعاية الطبية اليومية وفقا لـ 26 معيارا محددا، وقد وجدت أن المركز قد استوفى المعايير ليتم منحه “اعتماد التميز في الرعاية المتمحورة حول المريض”.

واشار السيد ناصر النعيمي الى ان مؤسسة حمد الطبية تضع برنامج الرعاية المتمحورة حول المريض في صدارة جهودها لتحسين الجودة وتحديد مجالات التحسين، كما يشجع هذا البرنامج فرق المؤسسة على إيجاد طرق مبتكرة لتعزيز تجارب المرضى وتعزيز التواصل والتفاعل مع المرضى والمجتمع لضمان تقديم أفضل رعاية ممكنة لهم.

وبين ان المؤسسة تتطلع قدما إلى تحقيق جميع مرافقها الأخرى هذا الإنجاز والانضمام لمركز الرعاية الطبية اليومية ومركز عناية في الحصول على هذا الاعتماد.

بدوره أوضح الدكتور خالد الجلهم المدير الطبي لمركز الرعاية الطبية اليومية أن المعايير التي استخدمتها منظمة بلينتري الدولية لتقييم مركز الرعاية الطبية اليومية تضمنت جميع مكونات تجربة الرعاية الصحية المتمحورة حول المريض، بما في ذلك جودة التفاعل مع المريض، ومستوى الوصول للمعلومات، ومشاركة الأسرة، وبيئة مرفق الرعاية الصحية.

واوضح ان مركز الرعاية الطبية اليومية يقدم رعاية فريدة من نوعها، حيث يقوم بعمليات جراحية نهارية، وتوفر الرعاية الجراحية والإكلينكية للمرضى الداخليين في مكان واحد مخصص، حيث أنه يمكن للعديد من المرضى التعافي في منازلهم بعد إجراء عملياتهم مع حرص المركز على ضمان تحقيق أفضل رعاية ممكنة لهم وتوفير الرعاية الخاصة بمتابعة حالاتهم.

ولفت الى ان الاعتماد الدولي يعتبر البرنامج الوحيد من نوعه الذي يعترف رسميا وعالميا بقدرة فريق مركز الرعاية الطبية اليومية على تقديم التميز فيما يتعلق بالرعاية المرتكزة على المريض.

ومن جانبها نوهت الدكتورة سوزان فرامبتون رئيس منظمة بلينتري الدولية بأن الاعتماد يشير إلى أن مركز الرعاية الطبية اليومية يعد منظمة يشترك فيها طاقم العمل مع المرضى والأسر، حيث يتم إعطاء الأولوية لراحة، وتمكين، ورفاهية المريض وعائلته كعناصر أساسية لتقديم خدمات طبية عالية الجودة.

ويعتبر إنشاء المجلس الاستشاري للمريض والأسرة بمركز الرعاية الطبية اليومية جزءا رئيسيا في تحقيق أفضل رعاية مرتكزة على المريض والحصول على هذا الاعتماد، ويجتمع المجلس بصورة منتظمة لتقديم المشورة فيما يتعلق بعدد من القضايا الرئيسية والتحسينات في المركز.

ومن جانبها ذكرت السيدة مريم العمادي عضو في المجلس الاستشاري للمريض والأسرة بمركز الرعاية الطبية اليومية أن المشاركة في هذه الرحلة المهمة للمركز حتى الحصول على الاعتماد الدولي كانت بمثابة تجربة رائعة لها.
