Geriatric-Dermatology Clinic at Rumailah Hospital Expands Care Services

Doha: The Geriatric-Dermatology (GeriDerm) clinic has expanded its patient care services by adding face-to-face consultations to virtual sessions for older patients with skin diseases in Qatar.

From January 2021, all new referrals to the clinic will be seen by an interdisciplinary team of dermatologists and geriatricians in a specialized Hamad Medical Corporation facility for the patient’s first appointment.

The ‘GeriDerm’ clinic was first launched in June 2020 as a virtual clinic providing specialized telemedicine-based care to cover the medical needs of older patients with dermatological problems. The provision of virtual and telephone consultations was initiated when the challenges of access to care due to the COVID-19 pandemic became more apparent.

The GeriDerm clinic had 329 virtual encounters with older patients from June-December 2020; with approximately 40 patients being seen in the clinic where face-to-face consultation was deemed essential.

Resuming face to face is in accordance with clinical guidelines and new patients are now receiving appointments to be seen by clinical experts in the Dermatology and Venereology Department or the Elderly Day Care Unit in Rumailah Hospital, as well as continue their ongoing care online. Follow up appointments will be arranged as either in-person or virtual consultation, depending on clinical need and patient preference.

Regarding the growing need for providing specialized dermatological care for older adults, Dr. Sarah Al Khawaga from HMC’s Dermatology and Venereology Department said “Skin disorders are more common in older adults, because the skin becomes is dryer (less oily), less elastic and thinner with advancing age, which can make it more susceptible to disease. The most common skin diseases among older people include eczema, skin infections, and pruritus (severely dry and itchy skin). People with underlying health conditions, such as diabetes, or those who take multiple medications, can also experience increased skin problems.”

“The COVID-19 pandemic last year provided the impetus for the establishment of the Geriatric Dermatology telemedicine clinic as an innovative approach to deliver ongoing care to older patients. The virtual service was welcomed by many patients and their families because it reduced the risk of infection through exposure outside their home; it was also more convenient for some patients with specific health conditions and limited transport means,” Dr. Al-Khawaga said.

“However, in light of the clinical need to physically examine certain patients, especially during first time appointments, HMCs clinical leadership team approved face-to-face consultations to enhance patient centered care,” she added.

For his part, Dr. Wasim Akram, Geriatric Consultant at Rumailah Hospital, said: “The goal for the GeriDerm clinic is to promote early detection of dermatological conditions, especially in high-risk patients aged 65 years and above. This clinic addresses one of the most vulnerable population groups during the current COVID-19 pandemic through the provision of clear pathways for proper triaging, referrals, and minimal exposure to infection.”

الدوحة: وسعت عيادة الأمراض الجلدية لكبار السن خدمات رعاية المرضى التي توفرها عبر إضافة استشارات طبية وجهاً لوجه ، إلى جانب الاستشارات الافتراضية للمرضى الأكبر سناً المصابين بأمراض جلدية ، وقالت إنه اعتباراً من يناير الجاري، سيتم توزيع كافة التحويلات الجديدة للعيادة لمتابعة الفرق متعددة الاختصاصات من أطباء الجلدية وأطباء الشيخوخة في مؤسسة حمد الطبية في الموعد الأول للمريض.

وتم إطلاق العيادة الأولى لأمراض الجلدية /الشيخوخة في يونيو 2020 كعيادة افتراضية تقدم استشارات متخصصة عبر الهاتف لتغطي احتياجات المرضى الأكبر سناً في الأمراض الجلدية، وجرى العمل بالاستشارات الطبية عبر الهاتف مع ظهور تحديات الحصول على الرعاية التي فرضها وباء (كوفيد-19). وقد أجرت العيادة الافتراضية 329 لقاءً مع كبار السن منذ يونيو وحتى ديسمبر الماضي ، بينما تم فحص نحو 40 مريضا وجهاً لوجه حيث استدعت حالاتهم ذلك.

وجاء استئناف خدمة الاستشارات الطبية وجهاً لوجه، وفقاً للتوجيهات الطبية ، بحيث يتلقى المرضى الجدد مواعيد ليتم فحصهم من قبل أطباء الجلدية بقسم الأمراض الجلدية أو بوحدة الرعاية اليومية لكبار السن بمستشفى الرميلة ، إلى جانب المواعيد الافتراضية المجدولة لهم على أن يتم تحديد مواعيد للمتابعة وجهاً لوجه أو افتراضياً وذلك حسب الحالة الصحية ورغبة المريض.

وعن تزايد الحاجة لتقديم خدمات متخصصة في أمراض الجلدية لكبار السن، أوضحت الدكتورة سارة الخواجة من قسم الأمراض الجلدية أن مشاكل الجلد تعتبر أمرا شائعا عند كبار السن، لأنه يصبح أكثر جفافاً ورقة وأقل مرونة مع التقدم في السن ما يجعله عرضة للأمراض مثل الاكزيما والتهابات الجلد والحكة، لافتة إلى أن الأمراض الجلدية تظهر بشكل أكبر عند المصابين بالسكري والأشخاص الذين يتناولون أدوية متعددة.

وقالت إنه مع ظهور وباء (كوفيد-19) العام الماضي، كان من الضروري إنشاء عيادة الاستشارات عبر الهاتف للأمراض الجلدية والشيخوخة والتي كانت بمثابة مسار مبتكر لاستمرار تقديم الرعاية لكبار السن ، في وقت لاقت فيه الخدمة الافتراضية ترحيباً من العديد من المرضى وأسرهم لأنها أسهمت في خفض خطر العدوى القائم عند مغادرة المنزل، وفي توفير عناء التنقل بالنسبة للمرضى المصابين بأمراض معينة أو ممن لديهم وسائل تنقل محدودة.

ونوهت أنه في ضوء الحاجة السريرية لفحص بعض المرضى، خاصة في الموعد الأول لهم، وافقت مؤسسة حمد الطبية على تقديم الاستشارات الطبية وجهاً لوجه لتعزيز الرعاية التي تتمحور حول المريض.

من جانبه قال الدكتور وسيم أكرم ، استشاري أمراض الشيخوخة بمستشفى الرميلة، إن الهدف من إنشاء عيادة للأمراض الجلدية / الشيخوخة هو للكشف عن الأمراض الجلدية عند المرضى الأكثر عرضة في عمر 65 فما فوق ، مشيرا إلى أنها تستهدف المجموعات الأكثر عرضة في ظل وباء (كوفيد-19) من خلال توفير مسارات واضحة بأولويات مناسبة وتحويلات طبية وأدنى مستوى من التعرض للعدوى.
