Moderna or Pfizer? Head-to-head study finds one is slightly more effective

A head-to-head study of the Moderna and Pfizer vaccines has determined the former to be slightly more effective against COVID-19, at least as far as the alpha and delta variants are concerned.

The results of the study, conducted by researchers with Harvard Medical School, the Harvard T.H. Chan School of Public Health, Brigham and Women’s Hospital and the Veterans Administration, were published in the New England Journal of Medicine this week.

Findings indicated that subjects who received the Moderna vaccine (mRNA-1273) were less likely to experience COVID-19 outcomes than those who received the Pfizer–BioNTech vaccine (BNT162b2) during periods of alpha- or delta-variant dominance. Specifically, when the alpha variant was predominant, researchers recorded 4.52 infections per every 1,000 study participants who received the Moderna vaccine, but 5.75 infections per 1,000 among those who received the Pfizer vaccine. During delta predominance, an additional 6.54 infections per 1,000 were recorded in the Pfizer group.

Moderna was also found to be slightly more effective against every category of COVID-19 outcome, including infection, symptomatic infection, hospitalization, ICU treatment or death.

Both of the vaccines, however, are highly effective against COVID-19 outcomes, the study’s authors said.

“Given the high effectiveness of both vaccines, either one is strongly recommended to any individual offered the choice between the two,” said lead author Barbra Dickerman, an instructor of epidemiology and investigator at the Harvard T.H. Chan School’s CAUSALab.

Dickerman acknowledged that while the differences in effectiveness were slight, “they may be meaningful for larger decision-making bodies, such as health care systems and higher-level organizations, when considering the large population scale at which these vaccines are deployed.”

The study, meanwhile, was conducted prior to the outbreak of the omicron variant, meaning more research is needed to assess either vaccine’s effectiveness against the latest mutations.

وتشير نتائج الدراسة، التي أجراها باحثون في كلية الطب بجامعة هارفارد، ومستشفى بريغهام وإدارة المحاربين القدامى، ونشرت في مجلة نيو إنغلاند للطب، إلى أن  الأشخاص الذين تلقوا لقاح موديرنا كانوا أقل عرضة للإصابة بكوفيد-19 ومضاعفاته، وحتى خطر الوفاة، بقليل.

ومن بين 3 ملايين و103 ألف و470 شخصا من المحاربين القدامى، الذين تلقوا جرعتهم الأولى من اللقاحات المختلفة المضادة لفيروس كورونا المستجد، كان 367 ألفا منهم قد تلقوا لقاح فايزر، فيما تلقى 397 ألفا لقاح موديرنا. 

واختارت الدراسة 219 ألفا و842 من الذين تلقوا لقاح فايزر، كما اختارت نفس العدد ممن تلقوا لقاح موديرنا، بين الفترة من الرابع من يناير الماضي والأول من يوليو، لإجراء الدراسة عليهم. 

وخلال تلك الفترة كان المتحور ألفا هو السائد، وسجل فيها الباحثون 4.52 إصابة لكل ألف مشارك في الدراسة تلقوا لقاح موديرنا، في حين أصيب 5.75 من كل ألف بين أولئك الذين تلقوا لقاح فايزر. 

وبلغ متوسط فترة المتابعة 126 يوما، أي على مدار 24 أسبوعا، تم فيها توثيق إدخال 258 شخصا المستشفى ممن تلقوا لقاح فايزر مقارنة بـ153 ممن تلقوا النظير موديرنا. 

وتم إيداع 77 شخصا ممن حصلوا على فايزر، العناية المركزة، مقارنة بـ48 ممن تلقوا موديرنا، فيما توفي 43 ممن تلقوا فايزر، مقارنة بـ38 ممن تلقوا موديرنا. 

وأجرى الباحثون أيضا الدراسة خلال فترة هيمنة المتحور دلتا، في الفترة من الأول من يوليو وحتى سبتمبر الماضي، مشيرين إلى أن نسب الإصابة بفيروس كورونا المستجد لم تتغير لدى الذين تلقوا لقاح موديرنا، وهي حوالي 4.52 من بين كل ألف شخص، في حين تم تسجيل إصابة 6.54 من بين كل ألف في المجموعة التي تلقت لقاح فايزر. 

وتشير الدراسة إلى أن كلا اللقاحين فعالين للغاية ضد فيروس كورونا، “إلا أن هذه النتائج قد تكون ذات مغزى لهيئات صنع القرار مثل أنظمة الرعاية الصحية وعند النظر إلى النطاق السكاني الذي يتم نشر فيه هذه اللقاحات”، بحسب المؤلفة الرئيسية باربرا ديكرمان، وهي أستاذة في علم الأوبئة، وباحثة في جامعة هارفارد. 

وأجريت الدراسة قبل بدء انتشار المتحور الجديد “أوميكرون”.. “مما يعني أن هناك حاجة إلى مزيد من البحث لتقييم فعالية كل من اللقاحين ضد الطفرات الأخيرة”، بحسب ديكرمان.
