Doha: The Official Gazette issued by the Ministry of Justice published in Issue No. 4 of 2023 the details of the decision, which includes 3 articles.
Article (1)
The following data must be present on the health insurance card in both Arabic and English:
1- Name of the card.
2- Name of the health insurance company.
3- Name of the beneficiary.
4- ID number.
5- Nationality.
6 Date of birth.
7 Number of beneficiaries.
8- Name and description of the beneficiaries.
9- Degree of insurance.
10- Card issuance date.
11- Signature of the cardholder.
12- Signature of the person in charge of the insurance company.
Article (2)
The insurance card must include the following specifications:
1- Recent photograph of its owner that enables the health care service provider to verify his identity and his entitlement to the health care services prescribed for him.
2- Bar code to read the card data electronically.
3- Logo of the insurance company.
The card shall be of a size that allows it to be easily carried and shown to the health care provider.
Article (3)
All competent authorities, each within its jurisdiction, shall implement this decision, and it shall come into effect from the day following the date of its publication in the Official Gazette.

الدوحة: نشرت الجريدة الرسمية التي تصدرها وزارة العدل في العدد رقم 4 لسنة 2023 تفاصيل القرار الذي يشمل 3 مواد.
مادة (1)
يجب أن تدون البيانات التالية في بطاقة التأمين الصحي باللغتين العربية والإنجليزية:
1- اسم البطاقة.
2- اسم شركة التأمين الصحي.
3- اسم المستفيد.
4- الرقم الشخصي.
5- الجنسية.
6 تاريخ الميلاد.
7 عدد المنتفعين.
8- اسم وصفة المنتفعين.
9- درجة التأمين.
10- تاريخ إصدار البطاقة.
11- توقيع حامل البطاقة.
12- توقيع المسؤول عن شركة التأمين.
مادة (2)
يجب أن تتضمن بطاقة التأمين على المواصفات التالية:
1- صورة فوتوغرافية حديثة لصاحبها تمكن مقدم خدمة الرعاية الصحية من التحقق من هويته واستحقاقه لخدمات الرعاية الصحية المقررة له.
2- الرمز الشريطي (الباركود) لقراءة بيانات البطاقة الكترونياً .
3- شعار شركة التأمين.
وتكون البطاقة بالحجم الذي يتيح سهولة حملها وإبرازها لمقدم خدمات الرعاية الصحية.
مادة (3)
على جميع الجهات المختصة، كل فيما يخصه، تنفيذ هذا القرار، ويُعمل به من اليوم التالي لتاريخ نشره في الجريدة الرسمية.
