HMC Introduces Nuclear Medicine for First Time to Treat Neuroendocrine Tumors

Doha: Hamad Medical Corporation (HMC), has introduced nuclear medicine in the treatment of patients with neuroendocrine tumors (177Lu-Dotatate therapy) for the first time in Qatar. The first patient successfully received treatment in the Nuclear Medicine Department at the National Center for Cancer Care and Research (NCCCR). This development eliminates the need for many patients to travel abroad for treatment.
Dr. Maryam Al Kuwari, Chief of Nuclear Medicine Radiology at HMC, emphasized that this treatment option is ideal for patients whose conditions have not responded to other treatments such as surgery, chemotherapy, and radiation therapy. It helps control symptoms, reduce tumor size, and manage its growth for many years, thus improving the quality of life for patients with neuroendocrine tumors. These tumors are often widespread in the body, making surgical removal difficult.
Dr. Maryam added, “We have been working for several years to achieve this medical milestone through a multidisciplinary team consisting of nuclear medicine physicians, medical physicists, oncology consultants, and nursing teams to provide the best possible medical care for patients. It is expected that approximately 10 patients per year will benefit from this type of treatment, which involves delivering radiation directly to cancer cells by binding the radioactive substance (Lutetium-177) to specific cancer-related proteins. When these proteins enter the body, they bind to cancer cells and deliver the radioactive substance directly to the tumor without causing harm to surrounding healthy tissues.” Dr. Maryam further stated, “We aim to offer more treatments like Lutetium-177 for other patients with conditions such as prostate cancer and joint diseases characterized by inflammation of the joint membrane due to iron and inflammatory factors after joint bleeding. As we expand the availability of these advanced treatments, the need for patients to travel abroad will significantly decrease.” Dr. Ghulam Syed, Head of the Nuclear Medicine Department at HMC, explained, “Radioactive nuclide therapy (nuclear medicine) has been available for a long time in the Molecular Imaging and Nuclear Medicine Department at Hamad Medical Corporation. Over the years, we have added more services, including low-dose iodine-131 therapy for patients diagnosed with thyroid gland hyperactivity and thyroid cancer, respectively. We also introduced microsphere therapy (Yttrium-90) for patients with unresectable liver cancers, whether primary or metastatic, and recently, Strontium-89 therapy to alleviate pain associated with cancer spread to the bones.” Dr. Ghulam mentioned that starting treatment may take nearly a month to complete blood tests, imaging studies, medication adjustments, and preparations for treatment. Typically, treatment consists of four sessions of injections, with one session every eight weeks. In most cases, symptoms improve after the second dose.

الدوحة: بدأت مؤسسة حمد الطبية استخدام الطب النووي في علاج مرضى أورام الغدد الصماء العصبية (العلاج باللوتيشيوم – 177 دوتاتيت) لأول مرة بدولة قطر.
وقد تلقى أول مريض العلاج بنجاح في قسم الطب النووي بالمركز الوطني لعلاج وأبحاث السرطان، مما يوفر على الكثير من المرضى عناء السفر لتلقي العلاج بالخارج.
وقالت الدكتورة مريم الكواري كبير أطباء أشعة الطب النووي بمؤسسة حمد الطبية: إن هذا النوع من العلاج يعد خيارا مثاليا للمرضى الذين توقفت حالاتهم عن الاستجابة للعلاجات الأخرى مثل العلاج الجراحي والكيميائي، وكذلك العلاج الإشعاعي، مما يساعد في السيطرة على الأعراض لدى المريض، وكذلك تقليص الورم والتحكم في نموه لسنوات عديدة، مما يساعد في تحسين جودة حياة مرضى أورام الغدد الصماء العصبية، خاصة أن هذه الأورام العصبية تنتشر في الجسم بشكل كبير مما يصعب إزالتها بالجراحة.
وأشارت إلى أنه تم العمل منذ عدة سنوات من أجل تحقيق هذا الإنجاز الطبي من خلال فريق متعدد التخصصات يضم أطباء واختصاصيي الطب النووي والفيزيائيين الطبيين، واستشاريي الأورام وفرق التمريض، في سبيل توفير أفضل رعاية طبية ممكنة للمرضى.
وتوقعت أن يستفيد من هذا النوع من العلاج حوالي 10 مرضى سنويا، حيث يتم تطبيقه عن طريق توصيل الإشعاع بشكل مباشر إلى الخلايا السرطانية، ودمج المادة المشعة (اللوتيشيوم – 177) مع البروتينات التي ترتبط بخلايا سرطانية معينة، مشيرة إلى أنه عند دخول البروتينات إلى الجسم، فإنها ترتبط بالخلايا السرطانية، وتقوم بتوصيل المادة المشعة مباشرة إلى الورم دون أن تتسبب في حدوث ضرر بالأنسجة السليمة المحيطة بالورم.
وأوضحت أن مؤسسة حمد الطبية تسعى إلى تقديم المزيد من العلاجات بتقنية (اللوتيشيوم – 177) لبقية المرضى الذين تتطلب حالاتهم الصحية ذلك، مثل سرطان البروستاتا وأمراض المفاصل التي تتميز بالتهاب الغشاء المفصلي الناجم عن الحديد والعوامل الالتهابية بعد نزيف المفاصل، حيث من المؤمل مع التوسع في تقديم هذه العلاجات المتقدمة أن تتراجع حاجة المرضى إلى السفر للخارج بشكل كبير.
ومن جانبه، أوضح الدكتور غلام سيد رئيس قسم الطب النووي بمؤسسة حمد الطبية أن العلاج بالنويدات المشعة (الطب النووي) متاح منذ فترة طويلة بقسم التصوير الجزيئي والطب النووي في مؤسسة حمد، وقد تمت إضافة المزيد من الخدمات على مر السنين، حيث بدأت الخدمة العلاجية بجرعة منخفضة من اليود 131 للمرضى، الذين تم تشخيص إصابتهم بفرط نشاط الغدة الدرقية وسرطان الغدة الدرقية على التوالي، ثم تم لاحقا إدخال العلاج بالكرات المجهرية “يوتريوم 90” للمرضى الذين يعانون من سرطانات الكبد غير القابلة للاستئصال، سواء الأولية أو المنتشرة في أنحاء الجسم.. كما يعد علاج “السترونتيوم – 89 ” لتخفيف الآلام المصاحبة لانتشار مرض السرطان في العظام أحد العلاجات المضافة مؤخرا في التصوير الجزيئي والطب النووي في مؤسسة حمد الطبية. ويواصل قسم الطب النووي بمؤسسة حمد الطبية، الذي يعد جزءا من التصوير السريري، مواكبة المعايير الدولية للممارسات الطبية لتقديم خدمات رعاية صحية عالية الجودة لكافة المرضى.
