Doha: Last year, a team of highly-skilled surgeons at Hamad Medical Corporation’s (HMC) Neurosciences Institute performed Qatar’s first endoscopic brain surgery to remove a high-grade tumor located deep in the center of a patient’s brain, and today the patient continues to be cancer-free. The patient, a 72-year-old Qatari female, was experiencing severe deterioration on arrival at the hospital and losing consciousness. Magnetic resonance imaging (MRI) of her brain showed the presence of a tumor in a difficult location of the brain; positioned deeper and more central than is usually seen. With the patient’s consent, Dr. Siraj Belkhair, Acting Head of the Neurosurgery Department at Hamad General Hospital, and his team decided to perform an endoscopic brain surgery procedure with a small wound incision less than three centimeters, as the general condition of the patient would not tolerate major brain surgery. The team was able to remove the tumor surgically by using an endoscope and neuro-navigation system that could precisely locate the tumor in the brain – the technique was done through MRI brain images. The results of the surgery exceeded all expectations and the recovery process, which included chemotherapy treatment at the National Center for Cancer Care and Research (NCCCR), was without complication. One year after the surgery, the cancer has completely disappeared and the patient is currently in good health and enjoying her life with her family and loved ones. “Endoscopic brain surgery is a procedure used primarily to treat brain tumors. It is considered a minimally invasive brain surgery that allows neurosurgeons to identify and treat conditions that are deep within the brain. This type of surgery allows us to treat brain tumors less invasively than traditional open brain surgery, while still affording the ability to get an in-depth view of the brain,” explained Dr. Belkhair. The technique used in this unique surgery was done in Hamad General Hospital by an expert neurosurgery team. The neurosurgeons used the images transmitted by the intraoperative neuro-navigation system as a guide for removing the tumor and creating a passage within the brain tissue to prevent the accumulation of brain fluid. The surgical team was equipped with the latest technologies and surgical instruments specialized in treating conditions that can affect the brain, such as tumors and lesions. “Endoscopic approaches have truly transformed the way brain tumors are treated, predominantly because endoscopic techniques give a clear visualization of the tumor, allowing surgeons to precisely localize them. These advanced techniques allow us to access the brain through pathways such as the nose and sinuses and do not limit us to only accessing the brain through the skull, which helps in removing tumors from deep and difficult areas in the brain without major complications,’’ said Dr. Belkhair. Commenting on the success of the procedure and the endoscopic technique used for the first time in Qatar, Dr. Ahmed Own, Head of the Neuroscience Institute at HMC said: “Endoscopic brain surgery represents a new milestone in HMC’s efforts to provide the very best cancer care and treatment in Qatar. أعلنت مؤسسة حمد الطبية عن تماثل مريضة سبعينية للشفاء التام من مرض السرطان، بعد خضوعها لجراحة دقيقة بالمؤسسة، عن طريق المنظار هي الأولى من نوعها في قطر، تم فيها استئصال ورم لمفومي خطير بمنطقة عميقة بمنتصف الدماغ. وقالت المؤسسة في بيان اليوم، إن الجراحة أجراها فريق جراحي بمعهد العلوم العصبية التابع لها، وأن المريضة “قطرية الجنسية” استمرت تحت الملاحظة والمتابعة الطبية على مدار عام كامل حتى تعافت تماما واختفت الخلايا السرطانية من الدماغ بشكل نهائي، مؤكدة أن المريضة تتمتع الآن بصحة جيدة وتمارس حياتها بشكل طبيعي برفقة عائلتها. وأشارت إلى أن المريضة كانت قد وصلت إلى المستشفى مصابة بأعراض تدهور حاد في حالتها الصحية وفقدان الوعي، وأظهرت نتائج فحص التصوير بالرنين المغناطيسي وجود ورم بالمخ في موقع يصعب الوصول له، كونه يقع في نقطة أكثر عمقا وأقرب لمنتصف الدماغ عما اعتاده الأطباء في الحالات المشابهة. وبعد الحصول على موافقة المريضة، قرر الدكتور سراج الدين بالخير، رئيس قسم جراحة الأعصاب بالوكالة بمستشفى حمد العام- وفريقه الجراحي إجراء عملية جراحية للمريضة بالمخ باستخدام المنظار من خلال شق جراحي صغير لا يتجاوز حجمه 3 سنتيمترات، حيث كانت الحالة الصحية العامة للمريضة لا تسمح بإجراء جراحة كبيرة بالمخ لها. ونوهت إلى أن الفريق الطبي تمكن خلال العملية الجراحية من استئصال الورم باستخدام المنظار ونظام توجيه مخصص لجراحات المخ والأعصاب أتاح له تحديد موقع الورم بدقة والوصول له بالاستعانة بصور الرنين المغناطيسي، لافتة إلى أن نتائج العملية الجراحية فاقت كل التوقعات. وأوضحت أن المريضة قد أتمت مراحل التعافي بعد الجراحة، والتي تضمنت العلاج الكيماوي بالمركز الوطني لعلاج وأبحاث السرطان، دون مضاعفات، لتظهر الفحوصات بعد عام من الجراحة، اختفاء الخلايا السرطانية بشكل كامل. وفي هذا الصدد اعتبر الدكتور بالخير هذه العملية فريدة من نوعها، مضيفا القول “تستخدم تقنيات جراحة المخ بالمنظار بشكل رئيسي لعلاج أورام المخ، وتتيح هذه العمليات -التي تتضمن إجراء بضع (شق) جراحي صغير الحجم، لجراحي المخ والأعصاب، تحديد مواقع الأورام التي تقع في مناطق عميقة بالدماغ بدقة وعلاجها، ويسمح لنا هذا الأسلوب الجراحي بعلاج أورام الدماغ من خلال شق جراحي أصغر من جراحات المخ التقليدية مع ضمان قدرتنا في الوقت نفسه على معاينة الدماغ بصورة متعمقة”. وبين أن الفريق الطبي استعان خلال الجراحة بأحدث التقنيات والأدوات الجراحية المستخدمة في مجال جراحة المخ والأعصاب لعلاج حالات أورام وإصابات الدماغ المختلفة، مشيرا إلى أن تقنيات جراحة المناظير ساهمت في تطوير طريقة معالجة أورام المخ بشكل كبير. وفي تعليقه على نجاح هذه العملية، قال الدكتور أحمد عون، رئيس معهد العلوم العصبية بمؤسسة حمد الطبية إنها المرة الأولى في قطر التي تستخدم فيها تقنية المنظار في هذه الجراحة، وأضاف “يمثل إجراء جراحات المخ بالمنظار إنجازا جديدا لمؤسسة حمد الطبية ضمن جهودها الهادفة لتوفير أفضل خدمات الرعاية والعلاج لمرضى السرطان في قطر”. كما يؤكد نجاح هذه الجراحة التزام الفريق بتقديم خدمة عالية الجودة مع التركيز على تعزيز صحة وسلامة المريض، وكذلك مواصلة مؤسسة حمد الطبية العمل على مكافحة مرض السرطان بكافة أنواعه، بما في ذلك أورام الدماغ من خلال تطبيق أحدث المنهجيات والتقنيات العلاجية.
