Al Wakra Hospital’s dental clinic treats more than 900 patients

DOHA: Qatar’s first dental clinic for patients with special needs, located in Al Wakra Hospital, has cared for over 900 children and adults with special needs since opening in 2015.

The first of its kind specialised clinic in the Middle East, has seen an increase in demand from patients under the age of 18 with special healthcare needs often have more severe dental problems.

The disability, and the increased incidence of anxiety that can accompany a physical or developmental disability often results in more complex care needs, said Dr Abdulhakim Ahmad Al Yafei, Senior Pediatric Consultant for Special Needs Dentistry and Head of the Dentistry Department at Al Wakra Hospital.

“Since the opening of our specialised clinic three years ago, we have seen an increase in the number of children who require our services. We are currently seeing around 20 children each week and these paediatric patients can have a variety of special requirements as a result of congenital or developmental conditions, or disease or trauma.

“Around 70 percent of the children we care for have autism, but we also care for patients with down syndrome, epilepsy, and multiple physical and developmental disabilities. To date, we have cared for over 200 children who have had multiple dental problems who we were able to treat in a single appointment through the use of sedation and general anesthesia,” Dr Al Yafei said.

Dr Al Yafei explained that sedation or general anesthesia is sometimes recommended in the case of patients who have multiple dental problems and are unable to sit in a dental chair. He said sedation allows for multiple procedures to be bundled together during the same appointment, which he says is less stressful for the patient because it means they only have to visit the clinic once for treatment.

He says it is often safer for patients with special needs to be sedated while procedures are carried out to protect them from the shape, fast-moving instruments required in dental treatment.

“We have recently begun using nitrous oxide, called laughing gas, in the treatment of many of our paediatric patients in particular, patients with Down syndrome. Nitrous oxide is a gas used to safely and effectively administer conscious sedation. During the administration of nitrous oxide, no bodily functions are affected and the patient remains in a comfortable state. The patient is awake but feels completely relaxed,” said Dr Al Yafei.

Al Wakra Hospital’s specialised dental clinic is fitted with state-of-the-art medical equipment and has a number of features designed to meet the needs of patients with physical disabilities, including a dedicated operating room, a special door which is wide enough to allow access to patients with mobility impairments, and a special lifting apparatus which can lift an object up to 400kg in weight.

This feature enables the treatment of patients while they are seated on their wheelchairs.

The clinic is staffed by one senior consultant and two specialists and provides routine dental care such as cleaning, checkups, tooth extractions, and fillings.

The clinic also provides care for patients who are unable to clean their own teeth. Unlike most HMC services, a referral from a primary healthcare or private clinic is not required.

استقبلت عيادة أسنان الأطفال ذوي الاحتياجات الخاصة بمستشفى الوكرة منذ افتتاحها عام 2015 وحتى الآن حوالي 900 مراجع من الأطفال ذوي الاحتياجات الخاصة، تتراوح أعمارهم منذ الولادة وحتى سن 18 عاماً، بمعدل حوالي 20 حالة أسبوعياً.

وأكد الدكتور عبد الحكيم اليافعي -رئيس إدارة طب الأسنان بمستشفى الوكرة، وزميل كلية الجراحين الملكية باسكتلندا مجال الأطفال والاحتياجات الخاصة- على أهمية دور هذه العيادة، قائلاً: «إن العيادة شهدت تزايداً في أعداد الأطفال المراجعين منذ افتتاحها؛ وأغلب الحالات من الأطفال ذوي التوحد؛ وتقدر نسبتهم بحوالي 70 % من إجمالي عدد الأطفال المراجعين، وقد خضع 110 أطفال منهم للتخدير الكامل لعلاج مشاكل متعددة بأسنانهم، حيث يتم علاجها جميعاً في نفس اليوم لكل طفل».

كما تستقبل العيادة أيضاً الأطفال ذوي متلازمة داون، والأطفال ذوي الاحتياجات المتعددة الذين يعانون من إعاقات جسدية وذهنية، وكذلك الأطفال المرضى الذين يعانون من أمراض عضوية أخرى بمختلف أجزاء الجسم مثل الكبد، الكلية والقلب؛ حيث يتم علاجهم من خلال فريق طبي متخصص؛ حيث تم علاج 90 حالة منهم تحت تخدير كامل.

الجدير بالذكر، أن هذه العيادة تعد الأولى من نوعها في المنطقة للتسهيل على المرضى من الأطفال وغيرهم من ذوي الاحتياجات الخاصة.

وأشار الدكتور اليافعي إلى أن العيادة مزودة بباب خاص ذي حجم مناسب يسمح بدخول المرضى ذوي الإعاقة الحركية، بالإضافة إلى حامل خاص يتيح علاج المريض دون الحاجة لنقله من كرسيه المتحرك، ويستوعب الحامل أوزان المرضى حتى 400 كيلو جرام مما قد يتيح إمكانية علاج المرضى كبار السن في حالة استحالة نقلهم، كما تم تجهيز غرفة عمليات تستقبل المرضى مرة واحدة أسبوعياً لعلاج الأسنان تحت التخدير الكامل، كما تتيح للمرضى الذين يجدون صعوبة عند إجراء الأشعة إمكانية إجرائها تحت التخدير الكامل أيضاً، كما أن وجود العيادة بالمستشفى يوفر إمكانية الاستعانة بأطباء في مختلف التخصصات وفقاً لما تتطلبه حالة المريض.

وأضاف الدكتور اليافعي قائلاً: «تم استخدام الغاز الضاحك مؤخراً في علاج الأطفال، الأمر الذي ساعد على تسهيل العلاج بشكل كبير وتقليل الحاجة لاستخدام التخدير الكامل أثناء العلاج خاصة مع أطفال متلازمة داون؛ حيث تم علاج ما يقرب من 30 % من المرضى الذين كانوا في قائمة الانتظار للخضوع لتخدير طبي كامل عن طريق استخدام الغاز الضاحك».;
